You should be able to copy addr.db.bak to addr.db (probably not necessary) and use the installer to install it.  I think that what you are seeing here is that when you deleted the csv file, pilotmanager assumed you wanted to delete all entries.  So they're gone.  The addr.db.bak file is in PalOS format, and needs to be installed directly on the PalmPilot.
