Have a look at http://linux.in-berlin.de/pia/
There you find my GUI X11-frontend for address databases from the Palm
Pilot. It's known to run on Linux, Solaris, FreeBSD, NetBSD and maybe
many other Unices.
It can import CSV data in the format like the one of "pilot-addresses"
(from pilot-link). You have to convert your format with a script of your
own to the one of "pilot-addresses" first.
